To reserve your spot on the tour please complete the form below. If you have more than one person that is going on the tour please complete this form for each person. Once you submit this form you will need print the tour agreement (presented on the next page) and mail it to Extreme Tornado Tours along with a deposit of $500.00 in order for your reservation to be held. Until this is received your registration is held in a pending status. If the tour fills up before this information is received your spot will be filled.
